DBS Renewals / Applications - 13 Booked Is Unlucky For Some . . .

DBS renewals / new applicants for Mid Shropshire League clubs & officials in 2017

Renewals or requests for new clearances are to be processed by County Welfare Officer Barry Jennings and his wife Lynne on the night of the League’s management committee meeting on Tuesday, December 6, 2016, at the Sir John Bayley Club in Wellington, from 7pm onwards.

AND THE LIST IS NOW CLOSED . . .


REQUIREMENTS

National Insurance Number – a MUST

Driving Licence – if you have one

Passport – if you have one

Proof of address – plus, if it has changed in the last 5 years, details of the previous address

COST: £3.50 per person to process

Original DBS / CRB certificate (for bowls) may speed up process on the night
